So, who saw the 'wardrobe malfunction'? I was at the VMAs (won a pair of tickets on the radio!) and got the full uncut version!

After Kanye's ridiculously long and rambling speech the schedule was in disarray, so they cut to host Miley backstage mid costume-change. She was covering her 'modesty' (what's left of it) with a black curtain but it slipped (or was dropped) and her entire left boob was exposed. The camera quickly panned away to other celebs but Miley's headset was still live and she said, quite clearly, "sorry, my tit it out". They cut back to Miley who was once again covered and looking slightly sheepish - but then you saw her think 'fuck it' and she sure enough dropped the curtain and showed us both boobs in full view!
